Repair of Detachement of the Chathal ligament




Abstract
Traumatic telecanthus is produced by two varieties of backward displacement of the bony structures and injury to the medial canthal tendon, lacrimal sac and the preseptal musels. Tramatic detachment of canthal ligament usually resulted functional and cosmetical problems.
We had 9 cases of canthal ligament injury who were admitted to Severance Hospital during the last 13 years from Jan., 1964 to Dec., 1976.
These patients were clinically analyzed and got following results.
1. The incidence of detachment of canthal ligament was 9 cases (3.2%) of total 274 case of the eye traumatic patients.
2. The incidence of combined injuries to detachment of canthal liagment was as follows facial and eyelid laceration in 9 cases (100%), nasal bone and orbital rim fracture in 7 cases (77.8%), injuries of lacrimal system in 5 cases (55. 6%) and rupture of the eyeballs in 2 cases (22. 2%).
3. Male was predominated over female by the ratio of 2 : 1.
4. The mean age was 22.2 years old.
5. Lest eye was prominated over right eye by the ratio of 2 : 1.
6. The most frequent causes was car acciaent in 7 cases (77.8%) of the total 9 cases.
7. The average time of operative treatment was 8 hours in primary repair and 10.8 years in secondary repair after trauma.
8. Anesthesia was under the general with endotracheal anesthesia in all 9cases.
9. The mean post-operative hospitalization, date was 20.7 days.
10. Post-operative complication, should in 4 cases (44.4%), which was consisted of blindness following perforation of the eyeballs in 2 cases, obstruction of the lacrimal system in 1 case and infectious cyst of the eyelid in 1 case.
